Affiliate System Access Request

Please complete the form below to request user access to Affiliate System. All requests must be reviewed and approved by your college's Designated Department Admin. If approved, you will receive e-mail notification that your account has been created.

Please note: this request form is for ASU administrative staff to request user access to process online affiliate forms. If you are an affiliated faculty member seeking to renew your affiliation, please contact your unit at ASU.

This is the username that you use to log into MyASU, not your 10-digit ASU employee number.

Email notification (on or off). If you choose "on," you will receive automatic email notification(s) whenever a form reaches a review level you are in. Note: If you are the originator, with email notification turned on, you will receive a notification any time a form you originate reaches any review level, including Notice of Final Approval.
